Aanmelden<\/strong>
U kunt zich aanmelden door telefonisch contact met mij op te nemen via 06-30543557. Wanneer ik niet in de gelegenheid ben om op te nemen, kunt u de voicemail inspreken met uw naam en telefoonnummer; ik bel u dan zo spoedig mogelijk terug.<\/p>\n\n\n\n

Daarnaast is het ook mogelijk om het contactformulier <\/a>in te vullen. Ik neem vervolgens telefonisch contact met u op voor een korte screening, waarin we uw hulpvraag bespreken en nagaan of behandeling binnen mijn praktijk passend is.<\/p>\n\n\n\n

Verwijzing<\/strong>
Voor vergoeding via de zorgverzekering heeft u een geldige verwijsbrief nodig van uw huisarts of een andere BIG\u2011geregistreerde verwijzer.<\/p>\n\n\n\n
